During your stay, Third Eye Health is your primary after-hours care team, giving you round-the-clock access to a board-certified clinician. Through our secure technology, you and your nurse connect directly with a clinician who can evaluate your condition and provide treatment in real time.
Our board-certified clinicians specialize in caring for older adults and have access to your facility’s records. They're experts in post-acute virtual care, working closely with your nursing team to deliver the same high-quality care you'd receive in person.
Families may be invited to join the visit by phone or video when appropriate. Your nurse starts a secure telehealth visit if symptoms arise, and can quickly connect with a clinician with whom they can securely text, share photos, and video chat. For residents, this type of care often means getting seen faster, and can help to avoid an unnecessary trip to the emergency room.
Services are covered under Medicare Part B, with consent obtained at admission.
